WEEK 5 Unit 5: Discussion 2-PEER RESPONNSE Anti-coagulant Therapy Hello Rose, I think your plan to start the patient on an anticoagulant makes sense. I wonder why choose one anticoagulant therapy over another. Why is wayfarin a good choice for this patient over other anticoagulants . So I started to think about what I know about these medications and how they might effect this particular patient .This patient has diabets, hypertensionand some unkonwn arrythmia. I know that diabetes effect kidney function and this patient has suffered for more than 20 years so their kidneys may be effected . Hypertension can cause bleeding when given with anticoagulants particularly intracranial hemorrhage. We cant take this risk. Warfarin is an anticoagulant that could cause this however oral anticagulats will not cause tis. Warfarin works best when there is a mechanical heart problem . This patient has an arythmia so the best anticoagulat would be an oral anticagulats such as eloquis. Eloquis also doesn't potentiate kidney issues. (January et al., 2019; Kearon et al., 2016).
