CASE STUDY OF ILANA 2 Description of Intervention The intervention chosen for Ilana’s case is cognitive behavioral therapy. According the the American Psychological Association, this intervention teaches individuals to identify and change unhelpful patterns of behavior and thought processes while also learning to replace them with healthy coping skills. It should be noted that this intervention can be used in a myriad of contexts for many different problems a client could be facing (APA, 2017). In a study by Duron- Figueroa, Cardenas-Lopez, and Quero (2020), CBT with prolonged exposure was used as an early intervention for acute stress disorder for Mexican earthquake victims. Researchers found that there was significant reduction of symptoms of trauma-related disorders after intervention, noting an absence of PTSD symptoms as well as major improvement or total remission of other symptoms noted by the participants (Duron-Figueroa, Cardenas-Lopez, and Quero, 2020). They note that based on the results, CBT using prolonged exposure reduces the symptoms of trauma- related disorders through the processing of traumatic memories, this is consistent with previous research (Duron-Figueroa, Cardenas-Lopez, and Quero, 2020). Limitations to be aware of is that this study focused solely on Mexican victims of a major 2017 earthquake, limiting the generalizability of the results. Additionally, there is concern with research regarding acute stress disorder; given the short time frame to qualify for a diagnosis, there are massive constraints when trying to conduct controlled studies. Furthermore, sample sizes are often small at best, limiting the generalizability of studies. One must also consider the various contexts trauma falls into; this study focused on natural disasters, limiting the generalizability of the results towards other types of trauma such as violence, death, etc.
CASE STUDY OF ILANA 3 Rationale In Ilana’s case, CBT is an appropriate intervention for her given the intense and multi- faceted array of emotions she is experiencing while grieving for her son and husband. Her sudden flashes of rage and anger, while also experiencing periods of shock and denial have been a recurring pattern for the past 4 weeks. She reports she finds herself “zoning out” multiple times a day and feels like she is losing control. She questions the afterlife, having been raised Buddhist but converted to Christianity when married; her parents have found peace, but she has not given her conflicting religious concerns. She struggles immensely with accepting what has happened to her family. When utilizing CBT for Ilana, the focus should be to help process her thoughts and emotions surrounding the aftermath of the death of her husband and son while also addressing her thoughts of the afterlife, possibly by finding a balance between her current Christian values and the Buddhist ones she was raised with. Explanation of Support In providing support for Ilana, we would first address her emotions and thoughts surrounding the death of her loved ones. This would be done by providing a safe space for her to begin having the tough conversation of their passing, allowing her to lay out on the table her thoughts surrounding the incident and her emotions that come from them. This allows us to gain a better insight into what her frame of mind is, identify potentially problematic patterns, and begin to address them. A journal would be suggested to help her process outside of sessions. Next, we would take the time to connect the dots between her emotions and specific aspects of the incident while working to reshape and identify ways of coping; we would come up with a set of goals she would want to reach through our time together.
