3 SUBJECT 5: Provide Support to People Living with Dementia The assessments in this subject assess the candidate’s practical knowledge and skills in relation to the following units of competency: CHCAGE011 - Provide Support to People Living with Dementia CHCCCS038 - Facilitate the Empowerment of People Receiving Support IMPORTANT Before you start the assessments in this subject, make sure you have: Carefully read the instructions provided in each assessment task in this subject. Printed hard copies of any required vocational workplace forms and templates so you may bring them with you at your vocational placement organisation/workplace. Discussed and reviewed the assessment tasks in this subject with your workplace supervisor. Coordinated with both your supervisor and assessor regarding schedules on when you can complete these tasks while being directly observed by the assessor in the workplace.
4 Instruction The goal of this assessment is to assess your practical knowledge and skills in the following: Preparing to provide support to people living with dementia Using effective communication strategies Supporting the person to participate in activities according to the individualised plan Using a strengths- based approach to meet the person’s needs Accessing Advocay Services and Complaints Mechanism Completing documentation Reflection The workplace assessment is divided into seven tasks: Task 1 Familiarizing with Individualized Plan and Person-Centred Care Task 2 Person Centred Planning Task 3 Facilitating an individual's engagement in activities through the use of efficient communication techniques Task 4 Strengths-Based Approach and Behavior Management Task 5 Accessing Advocacy Services and Complaints Mechanisms Task 6 Documentation Task 7 Reflection These tasks must be completed within the context of ageing support in a residential or community setting, including family homes. For each task there is a separate observation checklist for the assessor to evaluate your performance. Refer to the task objectives and steps provided below. For each task refer to the corresponding observation checklist for the skills you need to demonstrate while being observed by the assessor and submit any required documentation. Before starting this assessment, your assessor will also discuss these tasks with you, as well as for instructions and guidance for satisfactorily completing them. They will also organise the resources required for this assessment (listed below). Resources required for assessment: To complete this assessment, you will need access to the following: Ageing support workplace, e.g. residential or community setting, including family homes, which will allow them access to: o Workplace supervisor
5 o Clients living with dementia (at least two) o Individual support plans for clients living with dementia o Family and carers of clients living with dementia o Aids, devices/appliances, equipment used by the client (e.g. hearing aid, walking stick/cane, etc.) o Facilities or equipment used to provide individualised support to clients living with dementia. Note that this will depend on the support activities and strategies documented in the client’s support plans. For example: Venue or location for recreational activities (e.g. arts and crafts, gardening, etc.) Resources and materials used for the activities (e.g. paints, canvasses, brushes, gardening materials, etc.) o Facilities or equipment used to store and maintain documentation and reports, e.g. computer, filing cabinets, cloud storage, organisation intranet, etc. o Individual support documentation, e.g. progress notes, risk assessment records, other client records, etc. o Harmony Care policies and procedures for: Providing support to clients living with dementia Reporting and documentation Storage of information or record-keeping Providing Person-Centered Support to Individuals Living with Dementia Task Objective: To provide support to two different individuals living with dementia, using person-centered approaches while upholding their rights and dignity. This task encompasses using tailored communication strategies, supporting activities aligned with their needs, managing changed behavior to minimize adverse outcomes, and completing necessary documentation. Steps to take. Preliminary Steps In your discussion with your supervisor: 1. Select and name the two clients with dementia whom you will be assisting as part of this assessment. 2. Identify the family members, carers, or individuals designated by the clients themselves who will also be participating in their support and care. 3. Verify and ensure that you will be granted access to their individualized support plans Support for Person A
6 1. Review the Care Plans: Familiarize yourself with the individualized care plans for Person A. Note their specific preferences, needs, and any behavioral concerns. 2. Meet Person A: Approach Person A in a respectful and person-centered manner. Introduce yourself and engage in a conversation to establish rapport and trust. 3. Tailored Communication: Use communication strategies tailored to their needs. Ensure clear and simple language, maintain eye contact, and actively listen. Document any communication difficulties or preferences. 4. Activity Planning: Identify an activity that aligns with Person A’s interests and abilities, such as a reminiscence activity. Engage them in this activity while adapting your approach to their pace and comfort level. 5. Behavior Management: If person A displays any changed behaviors that may impact her or others (e.g., agitation), use appropriate strategies from her care plan to de-escalate the situation while ensuring their safety and well-being. 6. Documentation: After your interaction with Person A, complete a detailed report (Client Support Documentation template) summarizing your communication, the activity, any behavioral concerns, and your response. Ensure accurate and objective recording. 7. Repeat the steps for Person B Note: Ensure confidentiality and privacy are maintained throughout the task, and use pseudonyms for the individuals involved. Consult with your supervisor or trainer as needed for guidance and feedback. TASK 1 Familiarizing with Individualized Plan and Person-Centered Care Review the client’s individualised support plan. STEPS TO TAKE 1. While being observed by your assessor, access and review the individualised support/care plan of the client whom you will be supporting through Tasks 2 4. 2. Meet with your supervisor to confirm whether you have understood the individualised support/care plan correctly. 3. Complete the Workplace assessment task question 1 after completing the demonstration part. ASSESSMENT CRITERIA Practical knowledge of the client’s individualised support/care plan Practical skills relevant to confirming and clarifying the individualised support/care plan OBSERVATION FORM Before starting this task, review the Workplace Assessment Task 1, Observation Checklist 1.1 provided along with this workbook.
