- Verified person’s First, middle and last name. - Date of birth. - Social security number. - Verbally or in writing, screen the individual for sexual transmitted diseases, recent unprotected sexual contact with prostitute or individual who has numerous contacts with other individuals of an unprotected individual. - Verbally or in writing, screen the individual for blood components, transfection factors deficiencies, which are critical to the cascade of the clotting of blood after injury. - Screen/query the individual for all drug use which integral to problems with STD, Hepatitis, chronic pain, and use of drugs of abuse. - Screen the individual for any and all drugs which interfere with clotting of blood and would cause strokes or interferences with cascade of the clotting factors. - Screen the individual for any recent surgeries or injuries or blood loss. - If all the above are answered so the individual’s I.D. is verified answered appropriately for all the questions then have the person sit in blood drawing chair. - Cut pieces of tape, check the labels match individual I.D. for blood tubes and/or unit bags. - Label tubes and place in order sample tubes, and unit bags. -Blood
