Treatment Plan
Constructing a treatment and service plan for a client involves various components. Prior to starting a treatment plan the professional should carry out a client evaluation. The evaluation should determine the basis of the difficulty or issues and assess the back ground of the client. After the completion of the evaluation, a professional can start constructing an agreement to fulfill the needs of the client. The treatment plan should include goals that relate to the difficulties and issues the client is experiencing, this assessment will discuss the components of Mr. McCunes personalized treatment program.

The case management process consists of five parts: assessment, treatment planning, linking, advocacy, and monitoring. It is not a linear process and does not follow the order it is written up. There may be some back and forth and returning to certain points.
I have recently undertaken client work as part of the academic requirement of this course and have experienced first-hand the important role which self-reflection plays when a client’s description of their problems, struck a similar chord within my own life. The identification of this set of maladaptive behaviours and rules of thought have only been identified and addressed due to participation of this Cognitive Behaviour Therapy Postgraduate Diploma and my initial work as a Trainee Therapist.
Since people and customers to be specific come from different cultural backgrounds, considering and understanding the cultural background and practices of every market niche. In ensuring that the organizational activities do not conflict with the cultures of clients and that they suit the needs of the cultures, the following would be done by the organization;
The staff will be trained appropriately to fit their role in the program. The staff has a goal of being an effective staff in which they want to provide the best care for the suffering inmates. The top three health officials beginning with the psychiatrist, will be required to attend multiple mental health seminars in order to be an effective program. The staff will base their training off of moral standards such as the four basic rules by Suresh Bada Math which are “1. Protect, promote and endorse human rights of the prisoners. 2. Take care of the needs of the prisoners to the extent feasible within the prison environment. 3. identify patients who have health problems and get them the needed health care. 4. Pay special attention to the vulnerable
I believe that the intake form reviewed all necessary questions needed to determine the client’s social and medical history to develop personalized goals for the client’s individualized treatment plan. I will begin to discuss the areas that the intake form reviews in order to obtain the necessary information to develop a treatment plan for the client.

Throughout the class, many aspects of the curriculum were seen as useful in future social work practice. Before this course, in other course work, treatment planning and goal setting was often something that was reviewed briefly, yet within the coursework of the capstone seminar this matter had been gone over in more detail. Within this course, it was possible to gain more insight into a more balanced way of setting treatment goals that included conversations with the client concerning what they desired out of treatment in addition to what was needed. This aspect of the course was particularly useful when it came to working towards the development of the treatment plan with the client who was presented during the case presentation, Sarah.

Preliminary treatment plans are followed by a comprehensive, master treatment plan within a short time period (days to weeks). A comprehensive treatment plan is developed to manage the client's most debilitating symptoms by matching current needs in recovery to realistic and actionable goals. The master treatment plan is formalized after a comprehensive assessment and accumulated data and problem lists from any diagnostic or cognitive tests.
Case management can be traced back to 1890 -1910 during the Settlement House era where professionals guided families through public services, mobilized communities, and empowered people to seek a better quality of life. These early professionals pioneered the practice of building relationships, negotiating fractured services, and educating individuals, all critical aspects of case management today (Dziegielewski, 2013, p. 312).
I have decided to use just three main components that are, in my view integral for the success of, the 8-to-10 session care plan. In my opinion First contact, trust building, and life enhancing goals, should be considered as one component. To me this is essential because, it is the foundation to start an effective treatment plan. No one will truly trust or confide in you as a counselor, if you don’t make a good impression. This is important to me since how can you as a counselor expect someone to really, and I mean really open up to you aren’t warm and inviting, trustworthy, and willing to be goal oriented in their treatment plan. Secondly, the BECHRISTLIKE example set forth in out text book is an imperative step. Think of it this way you are meeting a stranger whom you may know nothing about.
The organization of the program begins with a comprehensive evaluation to diagnose any medical or biochemical origins of disease. The client is assigned to a therapist. The client is given an individualized treatment plan that focus on attainable short and long term goals. Ongoing care with the help of referral sources is provided, to the client, through discharge planning and facilitating a successful adjustment back to daily activates. Services are provided by a multidisciplinary team of mental health professionals led by a psychiatrist.
Treatment planning is a critical part of the counseling process. They act as blue prints for the counselor and client in what goals need to be met and changes that need to occur.The quality of a treatment plan determines the effectiveness of the outcome for the client. This paper will display a developed treatment plan for Sharon and the ethical and legal considerations in treatment planning for this client.
