Insurers are less likely to reimburse APRN’s in states that mandate physician supervision. Nurses need to push for reform of the regulations governing APRN;s. The ACA includes four initiatives designed to provide increasingly patient centered care. Primary care nurses could see job locations or job descriptions changes with the implementation of these initiatives. These initiatives include Accountable Care Organizations (ACO), Medical Health Home (MHH), Community Health Centers (CHC), and Nurse Managed Health Centers (NMHC). An ACO are groups of healthcare providers who work together to provide cost efficient care for Medicare patients. Nurses will help an ACO by functioning as a care coordinator of Quality Improvement Manager. Care coordinators will have to manage care with doctor offices, hospitals, rehab centers, and home settings, Quality Improvement Managers will focus on analyzing data and promoting evidence based practices. The MHH is a team of healthcare providers who supply healthcare services. The goal is to increase access to care, improve quality of care, and decrease costs. The main components to achieve these goals are health information technology and payment revision. Nurses will need to be competent with technological venues such as electronic medical records and be advocates for reform regarding payment regulations. An example of a CHC is Community Health Center, Inc.
The committee of the Robert Wood Johnson Foundation Initiative on the Future of Nursing did a substantial amount of work that eventually resulted in the "Future of Nursing: Leading Change, Advancing Health" report. Quite specifically, this organization was responsible for performing the research that many of the measures of this report were based on. The Committee of the Robert Wood Johnson Foundation Initiative on the Future of Nursing went to the Institute of Medicine and analyzed a number of different data that pertained to the healthcare industry, governmental entities (both at the state and federal levels) and nurses, in particular, before determining a number of specific recommendations that would ultimately provide a positive effect if not a radical transformation for the healthcare industry in the United States.
The Institute of Medicine’s 2010 report on The Future of Nursing: Leading Change, Advancing Health acknowledges the changing healthcare sector in the US and describes future vision of healthcare and the role of nurses to fulfill that vision. The United States always strives to provide affordable and quality healthcare to the entire population of the country. In order to achieve this goal an overall restructuring of the healthcare system was necessitated. Nurses are considered to be the central part of the healthcare system to provide high quality and safe patient care. Nursing in the US is the single largest segment of the healthcare workforce with almost 3 million nurses working in different areas across the county. The changing

With an estimated 64 million people having seen the Institute of Medicine’s reccomendations on the future of nursing within the first year of its release, it is arguably one of the most influential pieces of medical literature of the 21st century, leaving a lasting impact on healthcare and paving an innovative path forward for the nursing profession. The institute of Medicine (IOM) is a nonprofit, unaffiliated orginazition. Its purpose is to provide advice to the government and private sector in order to make an informed health decision. xi). The IOM stressed that “nurses have key roles to play as team members and leaders for a reformed and better-integrated, patient-centered health care system” (IOM, 2011, p. xi).
The (Institute of Medicine Report ,2010) pointed that there was need in the transformation of nursing
214) “The ACA and the need for APRNs, nurse faculty, and nurse researchers would have increased dramatically under any scenario.” (L R Cronenwett [RWJF Iniative on the Future of Nursing], 2010, table 1). “Not only must schools of nursing build their capacity to prepare more students. The impact of report on practice particularly in primary care is very huge. Over the past decades, we have realized, with every passing decade, nursing has become an increasingly integral part of the healthcare services, so that a future without large numbers of nurses is highly impossible to envision. We have also witnessed the drastic changes and advancement that have happened in the field of nursing over the past decades. Nurses have to change their current health care practice in order to meet the growth and also the goals of the IOM report.
“According to the IOM report, the nursing profession is the largest segment of the nation’s healthcare workforce. Nursing represents the largest sector of the health professions, with more than 3 million registered nurses in the United States.”
The Impact of the 2010 Institute of Medicine (IOM) Report on the Future of Nursing in Education, Practice and Leadership
In 2008, The Robert Wood Johnson Foundation (RWJF) and the Institute of Medicine (IOM) launched an initiative to assess and reform the nursing profession to address the complex unique nursing needs for the current world. The IOM report published in 2010 calls on nurses to take greater role in the healthcare system by taking leadership roles and obtaining high level education. The report envisions nurses to use their full potential and attain academic progression to redesign the healthcare profession in United States.

Primary care practice will be greatly impacted by health care reform. To provide better health care, the country will see a trend away from specialty to primary care and the avoidance of acute issues by delivering this care in the community setting. As a result, primary care physicians will be overwhelmed with the patient load as the number of the insured increases exponentially. The committee recognizes the scope of nursing practice that remains untapped and therefore the profession

Nursing practice is also one of the key messages addressed in the IOM report. The report suggests that “nurses should practice to full extent of their education and training”. Furthermore, “nurses should be full partners with physicians and other health professionals, in redesigning health care in the United States. This report has a great impact on nursing practice.
